  4. Cyclosporin is metabolised by CYP3A4 isoenzyme. Maudsley page 577 gives a list of substrates using the same CYP3A4 for their metabolism, which includes Citalopram. Citalopram uses the same isoenzyme CYP3A4 for its metabolism and compete with Cyclosporin and this would result in toxicity of unmetabolised cyclosporin. I would tick Citalopram as the least appropriate among the given list.
