A lady with a history of mild depression attends clinic and informs you she is pregnant. She is prescribed sertraline 50mg and has been in remission for 5 months. Which of the following would you recommend?

A.

Increased the dose of sertraline to 100mg and continue

B.

Withdraw the sertraline and monitor

C.

Stop the sertraline and switch to fluoxetine

D.

Reduce to sertraline 25mg and continue

E.

Withdraw the sertraline and commence citalopram

According to Maudsley - the chance of relapse is high in pregnancy. The recommendation is -"pateint who are already recieving antidepressant are best maintained on antidepreesant during and after pregnancy."

so I think answer is either C (As Maudsley says most evidence support fluoxetine safety)

or

B - if she is low risk
