Feedback on Part II Essay paper Autumn 2003

Please post your thoughts on this paper. If you can remember the essay titles, that'd be really appreciated!

Cant remember exact wording but here goes,

1. Psychotherapy services for the elderly

2. How would you reduce delayed discahrges as

presented to your trust manager

3. Construct a new diagnostic category with criteria on

the lines of PTSD and Panic disorder

ESSAY (exact wordings but no no.3)

1. PSYCHOTHERAPEUTIC APPROACHES SHOULD BE CONSIDERED IN THE CARE OF

THE OLDER ADULTS WITH MENTAL HEALTH PROBLEMS. DISCUSS THIS STATEMENT

CRITICALLY WITH REFERENCE TO THE RANGE AND EFFICACY OF PSYCHOLOGICAL

TREATMENT IN THE ELDERLY.

2. YOU ARE ASKED TO BRIEF YOUR SERVICES MANAGERS ON FACTORS INVOLVED IN

PROLONGING STAY ON ACUTE ADMISSIONS. OUTLINE YOUR RESPONSE AND

SUGGEST HOW THESE PROBLEMS COULD BE AMELIORATED.

None of the essays seemed topical. Do you think they just make up the titles while they are sitting in the pub?

Did any of you write the Q3?I found majority did q2 and few Q1.

I DID No.2 `COS IT WAS LESS RESTRICTIVE,AND THERE WERE MANY POINTS TO TALK ABOUT RELATED TO BED BLOCKING ISSUES.

I did number 3,

mainly because I knew nothing about the evidence base for psychotherapy in the ancients and number 2 looked like the woffle hell hole to end them all.

As far as I remember number 3 went something like this:

Describe the criteria that a new syndrome would need to satisfy in order to be added to the classification system of psychiatric disorders, discuss in relation to PTSD and social phobia.

On the whole the essays were not predictable by reading recent APT/BJPsych.

I went to Guildford,NB and BBR and had a list of about 15 essays including Schizophrenia, suicide, transcultural, bipolar, parenting, pervasive develpmental disorders , DSPD, cholinesterase inhibs and new mental health act in various combinations.

oh well...

at least Im well read now.

Hi,

I attempted Question 2 - it went something like this -

'Your service manager informs/tells you that patients with acute psychiatric admissions have prolonged hospital stays. &nbsp:lol:iscuss the reasons for this and what actions you would take to deal with this problem'.

Q1. was on psychotherapy services for the elderly, discussing with respect to the evidence base!

'older adults' does not necessarily mean elderly! Hats off to you for doing essay 3 -you must be a very clever bod. I couldnt have even started that - anything neurotic and i cant help switching off! Best of luck to one and all.

| put the the prolonged admission question to my my medical secretary and a staff

nurse today, and to my surprise they gave enough points to achieve a pass mark

within minutes. Perhaps thats why the college is heavy handed in the way the

exam is conducted who knows. Food for thought

I hope that wasnt inferring that medical secretaries and nurses arent as intelligent as doctors. Job status doesnt always correlate that well with intelligent. Heck dont we all know clever nurses and thick doctors - I know i do. I would have more respect for the College if they scrapped Part 2 (which rumour has they are going to anyway and bring in an exit exam) and brought in annual communication skills/clinical exams to boot out the minority of colleagues who are disrespectful and lacking in empathy for patients. Surely that would be a better means of quality control than the current paper which, lets face it, does not more to assess specialist psychiatric skills than ability to get the questions right on who wants to be a millionaire.

next time the essay question might well be

'As a consultant in general adult psychiatry you have just finished a ward round which you felt was disorganised and stressfull and you think you may be suffering from burn out. What are the factors that lead to staff burnout and how can you ameliorate this. use recent evidence to support your answer' :-/

:'( i did Q3 and on reflection feel like a complete arse, I did write a good essay for PTSD and social phobia being in the diagnostic system ie .....illness vs something drug companies have cooked up to sell their products; but i wonder if the college wanted us to write about how these criteria get operationalized ect...., can anyone remember the exact wording of the question :-/

Q.1 Psychotherapeutic approaches should be considered in the care of older adults with mental health problems. Discuss this statement critically with reference to teh range and efficacy of psychological treatments in the elderly.

Q.2 You are asked to brief your service managers on factors involved in prolonging hospital stay for acute psychiatric admissions. Outline your response and describe actions that may make a difference

Q.3 &nbsp:lol:iscuss teh criteria that should be met fro teh definition of a new syndrome in teh classification of psychiatric disorders. Illustrate your answer with particular ference to Post Traumatic Stress Disorder and Social Phobia

same here, satya. did Q3 and as I walked out of the room I thought .. gosh, should've waffled on Q2 instead, coz with Q2 provided you have decent points and structure you could probably pass.

you can never guess what the college wants you to write about. :o

I thought it was really brave of those who attempted q3!definetly will standout and get better marks for being different.My guess is atleast 70-80%attemted Q2,and examiners might get bored reading about revolving'doors' and 'bed'blockers!!

We dont attempt essay questions to entertain examiners for the fear that they would get bored. We like to attempt which is easier and which gains more points. Question 2 was a good question for those who wanted to attempt a question where one could bring in their clinical experience and not depend too much on studies or references.

I agree with you about'non entertainment'aspect of these 'objective'exams.However there is a subconscious process that takes into account the handwritng,the structure,the novelty,organistaion etc etc to the final marking.I am not sure whether different topics are corrected by different examiners.If so then it[attempting q1 or q3]might not make a difference.

anyway after that critical reveiw anything is better!! ;)
